Registration Forms

To register for swim lessons, please use our convenient online registration form. If you prefer, you may print out the following registration forms and bring it to the facility of your choice to register.

Remember there is NO registration fee and no obligation to start if we cannot find the class time that fits your needs.

Withdrawal Forms

Written withdrawal forms are due on the 15th of the month to apply to the next month. For example, if you want to withdrawal from classes at the end of August your withdrawal form is due on Aug. 15.

You may fill out the online withdrawal form below or the form is available at the front desk of every location.
